True, yet a bit unfortunate, the traditional approach to testing, both in the IT and non-IT environments, has been on ‘identifying’ defects and ‘correcting’ them. Thus, organisations approach this critical function of Quality Control (also known as Testing) in a very reactive mode.
The zero defect model focuses on defect prevention which aims to minimise the number of defects through a tightly integrated approach for testing upfront in the Software Development Life Cycle (SDLC) and focuses on First Time Right metrics.

The post production defects, reported by business users, result in high cost of correction and dissatisfaction. The higher the post production defects the more the effect on the bottom line of the organisation. Further the average cost per defect removal increases exponentially for the post production defects. An IEEE research paper indicates that the cost of correction of a defect induced during requirements gathering stage is 200 times higher, if detected and removed during the Integration testing phase of the SDLC.
